Most people are surprised by how gentle spinal decompression is. The table pulls slowly and releases, and the majority of patients describe a stretch across the lower back rather than pain. This guide explains what a session actually feels like, why some people feel sore the next day, and the signs that mean you should call your chiropractor instead of waiting.

The Short Answer

Spinal decompression is not usually painful. It is a slow, computer-controlled stretch, not a sudden pull. Most patients at our Miami office describe it as a mild traction feeling through the lower back or neck, and a fair number find it relaxing enough that they doze off during the session.

What you should expect is pressure and stretch, not sharp pain. Sharp pain during a session is not normal, and it is a reason to speak up immediately so the settings can be adjusted.

What A Session Actually Feels Like

Getting set up

You stay fully clothed. A harness fits around your hips and chest, and you lie on the table face up or face down depending on what is being treated.

The pull and release cycle

The table applies gentle traction, holds it, then eases off. That cycle repeats for the session. The force builds gradually rather than all at once, which is why it feels very different from someone pulling on your legs.

How long it lasts

A session generally runs around 20 minutes. You can stop it at any point using a hand-held switch, which many patients find reassuring on their first visit.

What people commonly report

  • A stretching or opening sensation in the lower back
  • Mild pressure where the harness sits
  • Relief of leg or nerve symptoms during the pull for some patients
  • Feeling relaxed, sometimes sleepy

Why Some People Feel Sore Afterwards

Mild soreness after the first session or two is common and is usually compared to the ache after a new workout. Muscles that have been guarding a painful area for months are being asked to lengthen, and they can respond with a dull ache for a day or so.

That soreness typically settles as your body adjusts over the first few sessions. It should trend downward, not upward.

Things that make soreness more likely

  • Going straight back to heavy lifting or intense exercise
  • Long periods sitting without breaks, including long drives
  • Skipping the home care your chiropractor gave you
  • Dehydration

Not Sure If What You Are Feeling Is Normal?

Talk it through with our team before your next session. We will look at your exam findings and adjust the plan if something is not sitting right.

Normal Versus Not Normal

Usually normal

  • Mild muscle ache for 24 to 48 hours after early sessions
  • Feeling tired after a session
  • Stiffness first thing in the morning that eases as you move
  • Soreness that gets a little better with each visit

Call us instead of waiting

  • Sharp or shooting pain during the session itself
  • Pain that is clearly worse after every session rather than easing
  • New or increasing numbness, tingling or weakness in a leg or arm
  • Any change in bladder or bowel control, or numbness around the groin — treat this as urgent and seek medical care immediately

That last point is rare, but it matters more than convenience. If it happens, do not wait for your next appointment.

What Makes It More Comfortable

  • Tell your chiropractor about discomfort during the session, not after — settings can be changed on the spot
  • Keep moving gently between sessions; walking is usually better than total rest
  • Break up long sitting, especially if you drive a lot around Miami
  • Drink water
  • Follow the home care plan you were given, including any stretches

Comfort is also about the plan fitting you. A session that is right for one patient may be too aggressive for another, which is why the exam and any digital X-rays matter before treatment starts.

When Spinal Decompression Is Not The Right Fit

Being honest about this matters more than selling a treatment. Spinal decompression is not appropriate for everyone. It is generally avoided or approached with caution in cases such as pregnancy, spinal fractures, advanced osteoporosis, spinal implants or hardware, tumours, or certain post-surgical spines.

A proper exam exists to catch these. If decompression is not right for you, there are other options — physical therapy, adjustments, laser therapy or shockwave therapy — and we would rather tell you that up front.

How Many Sessions Before It Feels Better

This varies a lot between patients and depends on what is driving the pain, how long it has been going on, and how closely the home care is followed. Some people notice a change early; others need to get through several sessions before the pattern shifts.

Your chiropractor should be reassessing as you go rather than committing you to a fixed number of visits regardless of how you respond.

FAQs About Spinal Decompression Discomfort

It should not. Most patients describe a slow stretch or mild pressure rather than pain. Sharp pain during a session is not normal — tell your chiropractor straight away so the settings can be adjusted.

Mild ache usually comes from muscles that have been guarding a painful area being asked to lengthen. It is similar to the soreness after a new workout and typically eases over the first few sessions. Pain that increases session after session should be reviewed.

Generally around 20 minutes. You stay fully clothed, and you can stop the table at any time with a hand-held switch.

Most patients can. Some feel a little tired or stiff right after a session, so if it is your first visit it is worth planning a little extra time rather than rushing straight into something demanding.

Heavy lifting, intense exercise, quick twisting and long stretches of sitting without breaks in the first day or two. Gentle walking is usually encouraged.

Yes. Our whole team speaks English and Spanish, and walk-ins are welcome, though appointments mean less waiting.
