When you search this question, you will see many different percentages. They disagree because they measure different things. One source counts a surgery as a success if leg pain improved. Another counts it only if the patient returned to full activity. A third looks at whether a second operation was needed years later.

The patients are different too. A person with a single herniated disc pressing on one nerve is not the same case as an older adult with narrowing at several levels of the spine. Grouping them together gives a number that does not describe either person well.

So the useful question is not what is the success rate in general. It is what is the likely result for my diagnosis, with this procedure, done by this surgeon. Only a spine surgeon who has examined you and reviewed your imaging can answer that.

What Spinal Decompression Surgery Actually Is

Decompression surgery is not one operation. It is a group of procedures with the same goal: to take pressure off a nerve or the spinal cord. The most common types are:

  • Microdiscectomy: removes the part of a herniated disc that is pressing on a nerve root. Often used for sciatica caused by a disc.
  • Laminectomy: removes part of the bone at the back of a vertebra to open space in the spinal canal. Often used for spinal stenosis.
  • Foraminotomy: widens the opening where a nerve leaves the spine.
  • Decompression with fusion: when the spine is unstable, the surgeon may also join two vertebrae together. This is a larger operation with a longer recovery.

Each of these has its own risks, recovery time and typical outcome. When you read a success rate online, check which procedure it refers to. A figure for microdiscectomy says little about a multi-level laminectomy with fusion.

What Affects Whether Surgery Works for You

Surgeons look at several factors when they estimate how a patient is likely to do. These are the ones worth understanding before your consultation:

How well your symptoms match your imaging

Surgery tends to help most when there is a clear link between what the MRI shows and what you feel. For example, a disc pressing on a specific nerve root, with pain running down the leg in the exact path of that nerve. Many people have disc bulges on an MRI that cause no symptoms at all, so an image alone is not a reason to operate.

Leg pain versus back pain

Decompression is designed to relieve pressure on nerves. It is generally more predictable for leg pain, numbness or weakness caused by a pinched nerve than for general low back pain without nerve symptoms.

Your overall health and habits

Smoking, diabetes, weight, bone density and previous spine surgery can all affect healing. How long a nerve has been compressed can also matter, which is why sudden or worsening weakness should be checked quickly.

What you expect afterward

Some people expect to be completely pain free. Others hope to walk to the store without stopping. Talk with your surgeon about which goal is realistic for you.

Questions to Ask Your Surgeon Before You Decide

A good surgeon will welcome these questions. Write them down and bring them to your appointment:

  • Which exact procedure are you recommending, and why that one?
  • What specific symptom do you expect it to improve: leg pain, back pain, numbness or weakness?
  • In patients like me, how often do you see that improvement, and how do you measure it?
  • What are the main risks, and how often do they happen in your practice?
  • What does recovery look like week by week, and when can I work and drive?
  • What happens if I wait, or try more conservative care first?
  • Is there a chance I would need a second surgery later?

If you feel rushed, or the answers stay vague, a second opinion from another spine surgeon is reasonable and common.

Non-Surgical Spinal Decompression Is a Different Treatment

Many people searching for spinal decompression surgery are actually reading about two different things with a similar name. Non-surgical spinal decompression is not an operation. It uses a computer-controlled table to apply gentle, gradual traction to the spine while you lie comfortably. There are no incisions, no anesthesia and no hospital stay.

The aim is to reduce pressure on discs and nerves and give the area a better chance to settle. It is usually done over a series of visits and is often combined with chiropractic care, Laser Therapy, physical therapy and home exercises.

It is not a replacement for surgery when surgery is truly needed. For some patients with disc-related back pain or sciatica who do not have red-flag symptoms, it can be a reasonable step to try before considering an operation. Whether it fits you depends on an exam, your history and, when needed, your imaging. We will tell you plainly if you are not a good candidate.

When Surgery or Urgent Care Is the Right Call

Conservative care is not right for everyone. Go to an emergency room right away if you have back pain with any of the following:

  • New loss of bladder or bowel control
  • Numbness in the groin, inner thighs or buttocks
  • Leg weakness that is getting worse, or a foot that drags
  • Back pain with fever, unexplained weight loss or a history of cancer
  • Pain after a serious fall or car accident, especially if you are older or have weak bones

These can be signs of severe nerve compression, infection, fracture or another condition that needs a medical doctor or surgeon, not a chiropractor.

Non-surgical spinal decompression is also not suitable for people who are pregnant, have spinal fusion hardware or other implants in the spine, have a recent spinal fracture, have severe osteoporosis, or have a spinal tumor or infection. If any of these apply, we will refer you to the right specialist instead of treating you.

And if you have already tried a full course of conservative care without meaningful progress, seeing a spine surgeon is a sensible next step. Choosing surgery after careful thought is not a failure.
