A stiff neck after sleeping usually comes from a neck muscle or joint that spent hours in an awkward position, often because of your pillow or sleep position. Most cases ease within a few days with gentle movement, heat or ice, and a better setup at night. Stiffness that comes with fever, a severe headache, numbness, weakness or a recent injury is different and needs medical attention. Here is how to tell the difference and what to do next.
Why your neck gets stiff overnight
During the day, you move your neck constantly, and your muscles keep shifting the load. During sleep, you can stay in one position for a long time. If that position holds your neck turned, bent or unsupported, the muscles and small joints on one side get strained. You feel it when you wake up and try to turn your head.
- A pillow that is too high, too flat or worn out. Your neck ends up bent up, down or to the side all night.
- Sleeping on your stomach. Your head has to stay turned to one side so you can breathe, which keeps the neck rotated for hours.
- Falling asleep somewhere else. A couch arm, a recliner, a car seat or an airplane seat rarely supports your neck.
- Strain from the day before. A hard workout, painting a ceiling, long hours at a desk or looking down at your phone can leave muscles tired, and the stiffness shows up the next morning.
- Stress and tension. Tight shoulders and a clenched jaw can carry into the night.
When something else is adding to it
Sometimes the pillow is only part of the story. Wear in the neck joints, a disc problem or an old injury can make your neck more sensitive to a bad night. If you wake up stiff most mornings, or the same side keeps locking up, there may be an underlying issue worth having examined.
One specific situation deserves a mention: stiffness that appears the morning after a car accident or a fall. Whiplash symptoms often show up hours or days later. Do not assume you slept wrong. Get checked.
What to do in the first day or two
For an ordinary slept-wrong neck, the goal is to calm the area and keep it moving. These steps are simple, and they work best together.
- Keep moving gently. Stay active within your comfort range. Holding your neck completely still for days tends to make stiffness last longer.
- Try heat or ice. Use whichever feels better. A warm shower or heating pad helps many people loosen up. An ice pack wrapped in a towel can help if the area feels sore or irritated. Use either one for about 15 minutes at a time, and do not sleep on a heating pad.
- Consider an over-the-counter pain reliever. If you can safely take one, follow the label. Check with your doctor or pharmacist first if you are pregnant, take blood thinners or have kidney, liver or stomach problems.
- Change how you do things. Turn your whole body instead of just your head, raise your screen to eye level, and take a break from heavy lifting for a day or two.
- Avoid forcing it. Do not crack your own neck, have someone yank on it or push hard into the painful spot.
Gentle stretches for a stiff neck
Once the sharpest pain settles, slow movement can help restore your range of motion. Work slowly and stop short of sharp pain. Do not bounce. Skip these and get checked if they cause numbness, tingling or pain that travels down your arm.
- Chin tuck. Sit tall. Draw your chin straight back, as if making a slight double chin. Hold for a few seconds, relax and repeat 5 times.
- Side tilt. Slowly lower your ear toward your shoulder without raising the shoulder. Hold for a few slow breaths, then switch sides.
- Slow rotation. Turn your head to look over one shoulder as far as is comfortable, pause, then turn to the other side.
- Shoulder rolls. Roll your shoulders up, back and down 5 to 10 times to release the muscles around the neck.
How to choose the right pillow
There is no single best pillow for everyone. The right one keeps your head and neck in line with the rest of your spine, and that depends mostly on how you sleep.
- Side sleepers usually need a firmer, thicker pillow that fills the space between the shoulder and the ear. Your nose should line up with the center of your chest.
- Back sleepers usually do better with a medium or thinner pillow that supports the curve of the neck without pushing the chin toward the chest.
- Stomach sleepers need the thinnest pillow possible, or none. A better goal is to move toward sleeping on your side or back.
What to look for when buying one
Material matters less than fit. Memory foam and latex hold their shape. Feather and down can be reshaped, but they tend to flatten overnight. Contoured or cervical pillows suit some people and feel awkward to others, so a return policy is useful.
Replace a pillow that stays flat, feels lumpy or no longer springs back. Avoid stacking two pillows, since that often pushes your neck too far forward. Give a new pillow a week or two before judging it, because your neck may need time to adjust.
Signs it is not just a slept-wrong neck
Most morning stiffness is minor. Some neck symptoms are not, and they should not wait for a chiropractic visit or any other office appointment.
Call 911 or go to the emergency room
- Stiff neck with fever, a severe headache, nausea, confusion or sensitivity to light. This can be a sign of meningitis.
- Sudden, severe neck pain or the worst headache of your life.
- Dizziness, double vision, trouble speaking, a drooping face, or weakness on one side of the body.
- Neck pain with chest pressure, shortness of breath, sweating or pain in the jaw or arm.
- Neck pain after a serious fall, car crash or blow to the head.
- New clumsiness, trouble walking or loss of bladder or bowel control.
See a doctor soon
- Numbness, tingling or weakness that travels into your arm or hand.
- Pain that keeps getting worse, or that has not improved after about a week.
- Pain that wakes you up at night, or stiffness that keeps coming back.
- Neck pain along with unexplained weight loss, a history of cancer, osteoporosis, a weakened immune system or long-term steroid use.
When chiropractic care is not the right choice
Chiropractic care is not right for every neck. If you have a recent fracture, severe osteoporosis, an unstable spine, a known problem with the arteries in your neck, or you take blood thinners, neck adjustments may not be appropriate. Tell your provider about these before any treatment. A careful chiropractor will examine you first, use a gentler approach when that makes sense, and refer you to a physician or an emergency room when your symptoms call for it.
How a chiropractor can help when it keeps happening
If your neck is not settling, or you keep waking up stiff, an exam can help find out why. Dr. Loucil starts by asking about your history, your sleep and your daily routine, then checks how your neck moves, how your muscles and nerves respond and whether you need imaging.
Depending on what the exam finds, care may include chiropractic adjustments, Laser Therapy, spinal decompression for certain disc problems, and physical therapy exercises to strengthen the muscles that support your neck. You will also get practical advice on your pillow, your sleep position and your work setup, since those are often what started the problem.
Results vary from person to person, and not every neck needs a long treatment plan. If you only need a few changes at home, you should hear that too.
Still stiff after a few days?
Dr. Loucil can examine your neck, explain what is causing the stiffness and tell you honestly what will help. Our office is in Ives Estates, close to Aventura.
Stiff Neck After Sleeping: Common Questions
Most stiff necks from sleeping wrong start to ease in a day or two and clear up within several days. If yours has not improved after about a week, or it is getting worse, have it examined.
Either can help, so use the one that feels better. Heat tends to relax tight muscles, and ice can calm a sore, irritated area. Use it for about 15 minutes at a time with a cloth between the pack and your skin.
Daily stiffness often points to your pillow, your sleep position or your mattress. If changing those does not help, joint wear, a disc problem, an old injury or muscle tension from daytime habits may be involved, and an exam can help sort that out.
Sleeping on your back or side is usually easiest on the neck, as long as your pillow keeps your head in line with your spine. Sleeping on your stomach is the hardest on it, because your head stays turned for hours.
Usually it is not, but it can be. Get emergency care if a stiff neck comes with fever, a severe headache, confusion, sensitivity to light, chest pressure, trouble speaking, or weakness on one side of the body.
For stiffness that lingers with no warning signs, a chiropractor is a reasonable place to start. If you have fever, nerve symptoms that are getting worse, a recent serious injury or a medical condition that affects your bones, see a physician first or go to the emergency room.
Yes, it could. Whiplash symptoms often appear hours or days after a crash. Get evaluated promptly, even if the accident seemed minor. Severe pain, numbness, weakness, or a headache that keeps getting worse needs emergency care.
